- Your name, address, date of birth, and email (for medical history, GP communication, billing, and appointment updates).
- Contact details for virtual sessions (e.g., Microsoft Teams, virtual platform access).
- Medical history to support effective diagnosis and treatment.
- Next of kin details for emergencies.
- Credit/debit card details for billing — these are never stored on file.
- Special category data (ethnicity, religion, biometrics, sexuality, etc.) recorded only if directly relevant to your care.
